- Copy of the foreign physical examination form (completed in Chinese or English). Note: Applicants must rigorously adhere to the Foreigner Physical Examination Form requirements. The “Foreigner Physical Examination Form” that is incomplete, lacking a photo of the applicant or a passenger seal, and not signed or stamped by a physician or hospital is invalid. The validity of the inspection result is six months. Please bring the original document when you register.
